Grade 8+ Salary Range $21.00-$23.64/hr. FLSA Non-Exempt
Basic Function: A Social Care Navigator helps individuals access necessary resources by conducting needs assessments, creating personalized action plans, and connecting clients to community services and programs within a social care network. Key duties include performing eligibility assessments, building relationships with clients and partner organizations, and using Social Care Network (SCN) platforms (Unite Us) to track referrals and outcomes, aiming to address social determinants of health like food insecurity and housing instability.
I. Qualifications
- Education/Experience
Bachelor’s or Associate’s degree in human service field (i.e. social work, psychology, nursing, rehabilitation, education, counseling, mental health, child and family studies, or sociology) and two years experience working directly with persons diagnosed with mental illness in an inpatient or community setting.
OR
Equivalent combination of education and experience.

- Ability to work independently and in a fast-paced environment.
- Proficiency in using computer equipment and software, including SCN platforms.
- Knowledge of Motivational Interviewing techniques.
- Ability to operate a vehicle legally in New York State.

III. Major Functions
- Needs Assessment: Conduct screenings and assessments using standardized tools to identify clients' social and health-related needs.
- Client Support & Goal Setting: Guide clients through the process of defining their needs, establishing goals, and developing personalized action plans to improve their well-being.
- Resource Navigation & Referral: Connect clients to relevant services and programs, both within the Social Care Network and through local, state, and federal resources.
- Referral Management: Utilize SCN platforms to initiate, track, and monitor referrals, ensuring effective and timely service delivery to meet client needs.
- Collaboration: Work with healthcare providers, community-based organizations, and other stakeholders to integrate services and overcome barriers to care.
- Data Collection & Reporting: Maintain accurate records of assessments, client interactions, and referral outcomes for program evaluation and quality improvement.
- Community Engagement: Build and foster relationships with community leaders and organizations to expand access to resources and promote health equity.
- Training: Attend required training and meetings related to program needs.
