Grade 8 Salary $19.95-$22.45 per hr. FLSA Non-Exempt
- QUALIFICATIONS
- Education/Experience:
- Bachelor’s degree in Human Services with 1 year of experience working with nutrition assistance programs and individuals facing food insecurity.
- Associates degree in Human Services with 3 years of experience working with nutrition assistance programs and individuals facing food insecurity.
- High School Diploma/GED with 5 years of experience working with nutrition assistance programs and individuals facing food insecurity.
4. Equivalent combination of education and experience.
- Requirements:
- Good oral and written communication skills.
- Demonstrated leadership ability and ability to work independently.
- Excellent knowledge of community resources for all populations.
- Demonstrated ability to establish rapport and relationships with clients and area agencies.
- Good interpersonal skills.
- Computer skills adequate to job duties; familiarity with electronic records and data collection.
- Knowledge of Broome County area.
- Valid NYS or PA driver’s license and ability to drive agency vehicle
- Must be able to lift over 40 lbs.
RESPONSIBLE TO: Pantry Supervisor
- MAJOR FUNCTIONS
- Administrative:
- Participate in staff meetings and agency trainings as required.
- Participate in monthly supervision with program supervisor.
- Responsible for case record documentation and daily and monthly record keeping.
- Represent the Agency and Program in a positive manner.
B. Client/Case Management Functions:
- Provide outreach services to individuals in the community in need of assistance obtaining entitlements to reduce food insecurity.
- Complete applications for assistance through public service programs/DSS with clients.
- Link clients to necessary services and act as an advocate.
- Complete individual needs assessments.
- Develop a person-centered plan with the client to obtain entitlements.
- Provide education and information to clients in need of nutritional supplement services.
- Work closely with local government offices to ensure that client applications are processed correctly.
- Provide clients with referrals to other nutrition assistance programs and community resources.
- Coordinate with other service providers and attend meetings as necessary.
- Attend community events to promote the program and represent the agency.
- Coordinate with supervisor and team members outreach activities on a weekly basis.
- Travel throughout Broome County to provide services in urban and rural communities.
