Salary range $17-$19.13/hr.                                              FLSA (Non-Exempt)


I.         QUALIFICATIONS:

Education/Experience

 
    1. High school diploma or equivalency
    2. Some experience in human service field

Skills

1.         Effective written and observational skills

bility to work with people at different functioning levels
5.         Good problem-solving skills
6.         
Ability to accommodate flexible work schedule
7.         Familiar with computer programs (e.g. Work, Excel)

 ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing up to 40 lbs.
9.          Ability to legally operate a vehicle in NY State



II.        RESPONSIBLE TO:            Supervisor

III.      MAJOR FUNCTIONS:

 
    1. Act as a role model while always providing supervision to residents
    2. Teach/assist in developing/completing ADL skills
    3. Perform routine household maintenance and upkeep
    4. Assist with planning/preparing meals and clean-up as needed
    5. Supervise resident’s self-administration of medications and/or treatments as per doctor’s orders and document in log if taken as prescribed
    6. Provide transportation as needed
    7. Assist in crisis management- crisis avoidance, de-escalation and intervention and follow resident’s behavioral support plan
    8. Participate in scheduled staff meetings, team planning meetings and case reviews
    9. Complete progress notes and documentation in daily log as required
    10. Assist resident’s in working towards achievement of service plan goals & objectives
    11. Understand and follow all program policies and procedures and OMH regulations
    12. Report all household or resident sensitive situations to supervisor
    13. Participate in and successfully complete all required staff training and development activities
    14. Participate in intake and discharge planning
    15. Participate in family meetings as requested
    16. Represent the Agency and program in the community and with other service providers
    17. Perform other related tasks as directed by supervisor
