Salary $24.15-$27.18/hr.
 
  1. QUALIFICATIONS
 
    1. Education
 
  1. Master’s degree in Human Services related field required (may include Credentialed Alcohol and Substance Abuse Counselor (CASAC), Creative Arts Therapist, Nurse Practitioner, Occupational Therapist, Pastoral Counselor, Psychologist, Registered Professional Nurse, Rehabilitation Counselor, Social Worker, Therapeutic Recreation Specialist, or Marriage and Family Therapist)
 
    1. Experience
 
      1. Minimum two years experience working with adult individuals living with a serious mental illness required.
      2. Preference given to individuals with experience on a multi-disciplinary mobile team.
 
    1. Skills
 
  1. Knowledge of mental illness, theory of etiology, Psychiatric Rehabilitation model and non-conventional treatment modalities.
  2. Ability to do functional assessments with individuals, clinical providers and family members.
  3. Ability to coordinate and participate in the development of an individualized, comprehensive service plan.
  4. Ability to work within a multidisciplinary team approach to outreach clinical and psychiatric rehabilitation model.
  5. Ability to interact with individuals living with serious mental illness in an effective and compassionate manner.
  6. Excellent organizational and communication skills.
  7. Good computer skills with working knowledge in the use of electronic health records necessary.
  8. Possession of a NYS driver’s license with good driving record.
 
  1. RESPONSIBLE TO:   Flex ACT Team Leader
 
  1. MAJOR FUNCTIONS:

    1. Consumer/Program Related
 
  1. Participate as a team member in the frequent monitoring and assessment of the mental health status of ACT enrolled individuals as well as related variables, including significant others, the home and the community in which the individual lives.
  2. Monitor and coordinate, along with the rest of the team, the delivery of necessary evaluations and assessments in order that each individuals needs are identified.
  3. Monitor and coordinate the delivery of necessary services by transporting or arranging transportation for individuals; assisting in making appointments; assisting in interactions with landlords, law enforcement, social services, employers, doctors, etc. per their service plan.
  4. Provide clinical expertise to team and provide clinical treatment to individuals utilizing techniques such as such as Cognitive behavioral therapy (CBT) and Dialectical behavior therapy (DBT).
  5. Develop a therapeutic relationship with individuals and assist them with problem solving and crisis management. 
  6. Assist individuals with developing social and community supports.
  7. Provide service coordination for Flexible ACT enrolled individuals.
  8. Maintain clinical record/chart manager for individuals including EMP, service plans every 6 months, progress notes addressing areas of the service plan weekly, updating of releases, and comprehensive assessments.
  9. Coordinate weekly recreational activities for consumers.
  10. Submit monthly reports and other required reports, and associated Medicaid billing.
  11. Attend daily team meetings and updates team on individuals status.
  12. Rotate assignment of weekly and weekend after hours on call coverage.

    2. Maintain awareness of community resources relevant to individual needs.
    3. Attend relevant trainings and professional meetings as required.
