Grade 9  Salary $22.05-$24.82/hr.                                                    FLSA Non-Exempt

 
  1. Qualifications:
 
    1. Education / Experience
 
  1. Bachelor’s degree in human services or related field required.
  2. Minimum two years’ experience in behavioral health.
  3. One year’s experience in substance use setting preferred.
 
    1. Skills
 
      1. Knowledge of mental illness diagnosis and wellness strategies
      2. Knowledge of Dual Disorders
      3. Ability to work within a multidisciplinary team
      4. Good organizational and time management skills
      5. Good oral/written communication skills
      6. Good interpersonal skills.
      7. Knowledge of community agencies and resources
      8. Ability to work with service providers.
      9. Familiar with computer programs and electronic health records.
      10. The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 25 lbs.
      11.  Ability to legally operate a vehicle in NY State
 
  1. Responsibile to:   Flexible ACT Team Leader
 
  1. Major Functions:
 
    1. Program
 
    1. Monitor and assess individual Mental Health/wellness status.
    2. Identify individual needs based on ongoing assessments.
    3. Develop a therapeutic relationship with individuals and assist with problem solving and crisis management.
    4. Educate individual and/or family on diagnosis/symptom recognition.
    5. Conduct care planning meetings with all relevant service providers.
    6. Act as a linkage to community services including medical, behavioral, residential, services, etc.
    7. Utilize software technology pertinent to program.
    8. Maintain required documentation and deadlines.
    9. Ensure that a person-centered planning process is utilized in delivery of services.
    10. Link and connect to natural supports and community resources
    11. Ensure that individual rights are protected and advocate for individual
    12. Regular and substantial contact with individuals receiving services
    13. Participates as a team member in the frequent monitoring, assessment and evaluation of the mental health status for individuals; including significant others, the home and the community in which the individuals live.
    14. Monitors and coordinates the delivery of necessary services including transportation, making appointments, assistance with housing, law enforcement, social services, employers and doctors.
    15. Develops a therapeutic relationship with individuals and assists them with problem solving, crisis management and the development of social support.
    16. Provides service coordination/chart management for 10 individuals.
    17. Maintains accurate and up to date records for individuals enrolled in ACT.
    18. Attends daily team meetings and provides regular updates on status of individuals.
    19. Rotates assignment of weekly and weekend after hour’s coverage.
    20. Provides technical assistance and education to members of the team in the area of wellness.  Develop materials and strategies for wellness self-management and other wellness interventions in conjunction with the team.

  2. Maintains awareness of community resources relevant to individual needs. 
  3. Attends relevant training and professional meetings and represents agency business in professional manner.
