Salary $24.25-$27.18/hr. FLSA (Non-Exempt)
- QUALIFICATIONS:
Education/Experience
- Master’s degree in a Human Service field required
- Knowledge of treatment options for individuals with Dual Diagnosis
- Minimum of three years’ experience working in Behavioral Health
Skills
- Knowledge of mental illness diagnosis
- Knowledge of Dual Disorders
- Ability to work within a multidisciplinary team
- Good organizational and time management skills
- Good oral/written communication skills
- Good interpersonal skills
- Knowledge of community agencies and resources
- Ability to work with service providers
- Familiar with computer programs (e.g. Word, Excel)
- The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 40 lbs.
- Ability to legally operate a vehicle in NY State
- RESPONSIBLE TO: Supervisor
- MAJOR FUNCTIONS:
- Monitor and assess individual Mental Health status
- Identify individual needs based on ongoing assessments
- Develop a therapeutic relationship with individual and assisting with problem solving and crisis management
- Educate individual and/or family on diagnosis/symptom recognition
- Conduct care plan meetings with all relevant service providers
- Act as a linkage to community services including medical, behavioral, residential, services, etc.
- Utilize software technology pertinent to program
- Maintain required documentation and deadlines
- Ensure that a person centered planning process is utilized in delivery of services
- Link and connect to natural supports and community resources
- Provide crisis intervention
- Ensure that individual rights are protected and advocate for individual
- Participate in and successfully complete all required training and development
- Represent the agency and program in the community and with other service providers
- Regular and substantial contact with individuals receiving services
- Actively participate in team meetings
- Perform other job related activities as directed by supervisor
- Understand and follow all program policies and procedures
